NYC Health Department Birth and Death Certificates

I have attempted through a court order from a judge in Illinois to have my daughter's birth certificate corrected because her father put a alise name on her certificate. This is the third time attempting this I am so frustrated to say the least!!!! They say I am missing this and that I send in what they want and they say I am missing something else!!!! I have a CERTIFIED COURT ORDER FROM A JUDGE!!!!!! Called today 11-17-15 at 9:30AM Spoke to a lady regarding this frustrating matter. She said a SUPERVISOR will call me back waited all day no one called me back. Why is it taking so long for my daughter's birth certificate to be corrected to my maiden name especially since I have a CERTIFIED court order from a judge in illinois which I paid for to be changed! It took me months to see a judge here in Illinois. I relocated back in 2013. Now that the judge order the name change from NYC there is still NO RESULTS! Come on NYC HEALTH DEPARTMENT CORRECTION UNIT get it together I was a resident of Brooklyn NY for 25 long years. GET IT TOGETHER ALREADY! PISSED OFF! Today 11-18-15 and still NO call from anyone 3 days and noone has called be regarding my daughter's birth certificate for a name change. I called twice before I was able to speak to a SUPERVISOR name James Serrocky. He was really nice and did his best to help me. But still unsure of what they want going to see the judge in Illinois for the fourth time on 11-30-15. Going crazy about now HELP!!!! A SUPERVISOR. name Miss Timberson called me today 11-19-15. She was EXCEPTIONAL she kindly explained everything to me of what was needed to get a new birth certificate for my daughter. I will be going before the same judge a fourth time on 11-30-15. Hopefully, I will have the new certificate from NYC HEALTH DEPARTMENT by the New Year of 2016. Thank you for the phone call Miss Timberson.

Horrible experience attempting to contact a real person. The automated system could not handle my situation. In California and need BC for a cruise that i have already paid for.

DONT FORGET YOUR MONEY ORDER $17.75!!!! Or a debit card. Makes the trip that much easier. You also need your photo ID. After that it's sort of like DMV. Type in your info, get a ticket with a number on it, and wait until your number is called. Go up to the window get your Birth certificate and your out the door.
