I had a good experience with them. I called and they came the next day to measure a pane of glass that needed to be replaced in a French door, and took measurements for glass tops for a desk and round side-table. They were polite quick, and called later that day with the price, which was very reasonable. Next day they showed up early, replaced the pane and made sure the table tops worked and left. They actually were less expensive than another glass shop that required that I make the table top measurements, and Forma included little rubber glass feet for the tops, which the other company did not. You may need to follow up with a call, but I found them to be very responsive.

Forma Glass is one of the most UNPROFESSIONAL establishments I have ever had the displeasure of coming in contact with. After Forma telling us that they could do the job and It took them 2 weeks to get back to us about pricing (for a VERY large job). Called multiple times and the receptionist told us that Anastasia would call back in 5 minutes. I would trust my 5 year old niece to do any glass job before them.
