i have been going for years and always paid cash, then got a bill for the copay and a dunning notice that i would be reported to a credit agency if i didn't pay the $15; i called to protest that i was always asked for payment as soon as i arrived and announced myself at the reception desk; the billing person confirmed that i always paid cash but didn't care that i got the notice and was going to report me so i am paying under threat but not because i owe it
